Requesting Plymouth Birth Certificates

Hennepin County gives you three ways to request a Plymouth birth certificate: in person at a service center, by mail, or by fax. In-person requests are handled the same day at either the Ridgedale or Maple Grove locations, or at any of the other four Hennepin service centers. Walk in during business hours and staff will process your request on the spot.

Mail requests go to the Hennepin County Vital Records main office at the Government Center: 300 South 6th St, Suite A025, Minneapolis, MN 55487-0026. Send your completed application form, a copy of your photo ID, and a check or money order for the fee. The county's birth certificate application is available as a downloadable PDF on the Hennepin County website at hennepin.us. Fax requests go to 612-348-2010 with the same materials plus credit card payment information.

For online requests, use the Minnesota Department of Health's VitalChek portal at vitalchek.com. The MDH order page at health.state.mn.us/birthnc covers the full online ordering process. Online orders include a VitalChek service fee in addition to the Hennepin County certificate fee.

Plymouth Birth Certificate Fees

Hennepin County charges $26 for the first certified copy of any birth certificate, including those for Plymouth births. Each additional certified copy ordered at the same time costs $19. If you want expedited processing, add $20 to the standard fee. These fees apply whether you request in person, by mail, or by fax.

Noncertified informational copies cost less and are available to requestors who do not qualify for a certified copy under Minn. Stat. 144.2255. These copies show the same data but are not valid for official purposes. Payment at the service centers can be made by cash, check, or card. Mail requests require check or money order. The county will not process requests without payment included.

Who Has Access to Plymouth Birth Records

Minnesota limits who can get a certified birth certificate. Under Minn. Stat. 144.2255, eligible requestors include the person named on the record (if age 18 or older), a parent or legal guardian, an attorney or legal representative with documented authority, and others who can show a valid legal need such as an estate executor. Valid photo ID is required for all requests. Staff may ask you to explain your relationship to the person on the record.

Adoptees wanting their original birth certificate go through the Minnesota Department of Health rather than the county office. Information on that process is at health.state.mn.us/adoption. If you are a homeless youth and need a birth certificate, the MDH has a free or low-cost assistance program described at health.state.mn.us/homelessyouth.

The Minnesota Department of Health vital records program at health.state.mn.us covers birth certificates for all Minnesota locations including Plymouth, and serves as an alternative to the county office for online and mail requests.
