The Ridgedale location is inside the Ridgedale shopping area off Interstate 394. Parking is free and plentiful. Bring your completed application, valid photo ID, and payment when you visit. The Maple Grove location is off Interstate 94 and is also easy to reach by car from northern Plymouth.
Requesting Plymouth Birth Certificates

Plymouth Birth Certificate Fees
