In-person requests are handled the same day in most cases. Visit 612 N Broadway in Crookston during business hours. Bring a valid photo ID and be ready to provide the name on the certificate, the date and place of birth, and your relationship to the person named. Staff will search the record, make a certified copy, and take payment before you leave.

Mail requests are accepted. Write a letter with all relevant details: the full name on the record, the birth date, the location of birth, your name and mailing address, your relationship to the person named, and a photocopy of your ID. Polk County Birth Record Fees

Polk County follows the statewide fee schedule for vital records set by Minn. Stat. § 144.2255. A certified birth certificate costs $26 for the first copy. Each additional certified copy of the same record ordered at the same time is $19. A non-certified copy costs $13 for the first copy and $6 for each additional copy ordered at the same time. Non-certified copies are informational only and not valid as legal identification documents.

Accepted payment types include cash, check, money order, and credit card. For mail requests, use a check or money order. Do not send cash by mail. Credit card payments can be made at the counter in person. Fees are not refunded if no record is found, as the fee covers the search as well as the copy itself.
